Getting Started

In order to determine insurance eligibility and programming needs, Allay will collect some general information from you. By clicking the link below, you will be directed to an intake form. After filling out the form, please press SUBMIT and we will contact you within 1-2 business days. 

Payment Methods

Allay is in network with and accepts the following insurances:

  • Florida Medicaid (Includes Sunshine Health)

  • Blue Cross Blue Shield

  • Cigna​

  • Aetna

  • Optum

  • United Healthcare

After submitting the intake form, we will determine your eligibility for insurance coverage. Upon completion of the intake form, we will also need a few documents from you in order to receive authorization to provide services. These documents are listed below. 

  • A diagnostic report from a psychologist or psychiatrist

  • Copies of the front and back of your insurance card

  • A prescription for ABA services from a pediatrician or primary care physician, or other healthcare provider
